Enrollment Forms

The following forms are all those necessary for your child to begin their time at a KIDS INC. Center. These forms include:

-Enrollment Form – Outlines pickup times and parental and staff responsibilities.
-Pickup Authorization – Specifies who is allowed to pick the child up from KIDS INC.
- Assessment Form – A general overview of the childs health history and allergies and the immunizations the child has had.
-Emergency Contact Form – Outlines who should be contacted if an emergency should occur and the initial contact cannot be reached.
-General Handbook – Outlines rules and regulations for staff, children, and parents.
-Immunization Form – Specifies what vaccines the child has had and when.

*As of August 22, 2016, all KIDS INC. centers, summer programs, and school-age programs will no longer accept Block Grant applicants.
